To our Valued Passport Patients
Unfortunately, due to circumstances beyond our control, Passport has informed Harrison Memorial Hospital and HMH Physician Group that they are terminating our contract effective October 1, 2019.
You are welcome to stay in Passport. However, you would not be able to be seen at HMH Physician Group: Primary Care Office, HMH Physician Group: Obstetrics and Gynecology Offices or the Clinic at Walmart operated by Harrison Memorial Hospital.
If you would like to continue seeing your provider, the state will allow you to change their Managed Care Organization through a process called “Disenrollment for Cause”.
In order to start the process, you will need to complete a form and fax it to the Cabinet for Health and Family Services at 502-564-0509 by FRIDAY, SEPTEMBER 20. Here is a link to the form to print: https://chfs.ky.gov/agenci…/…/member/Pages/disenrolment.aspx
Forms are available in the HMH Physician Group: Primary Care Office and HMH Physician Group: Obstetrics/Gynecology Offices. Those providers are: Dr. Shane Gainey, Dixie Stone, Eugonda Fryman, Stacy Gilkey and Daniel Klaber for HMH Physician Group: Primary Care. For HMH Physician Group: Obstetrics/Gynecology offices, those providers are Dr. Gerald Harpel, Dr. Derek Clarke and Dr. Laura Moore. HMH will assist in faxing the information to the Cabinet of Health and Family Services.
Please meet with Diane Moore or Chris Harrison at the HMH Physician Group: Primary Office; Meredith Furnish at Drs. Clarke and Moore’s office and Melissa Hutson at Dr. Harpel’s office.
In addition, the HMH Financial Counselor, Joy Baker, is available to assist. Her office is located on the ground level next to the Emergency Department. Also, the HMH Front Lobby desk has forms available as well.
Harrison Memorial Hospital and HMH Physician Group will continue to participate with the other Kentucky Medicaid Managed Care Organizations: Aetna Better Health, Wellcare of Kentucky, Anthem, and Humana Caresource.
If your primary care physician is with another office outside of HMH Physician Group: Primary Care office, please contact the office to make arrangements.
