Open Admissions Policy
All services and facilities of the hospital are available to patients and visitors without discrimination as to sex, race, color, creed, religion, payment source, national origin and source of payment and are readily accessible to the disabled.
Your Rights
As a patient of HMH, be assured that we value and protect your rights. All employees, physicians, and volunteers recognize and support these rights.
You have the right to:
- All services and facilities of the hospital without discrimination as to sex, race, color, creed, religion, payment source or national origin.
- Considerate and respectful care, including timely response and management of your pain.
- Complete and current information concerning your diagnosis, treatment, and care needs. This includes being able to view your medical record with a hospital member present.
- Necessary information to make informed decisions and give consent prior to any treatment or procedure. This information includes risks, benefits, and any alternatives that may be available to you.
- Refuse treatment (to extent permitted by law).
- Privacy concerning any care discussion, examination, and treatment.
- Expect that all communication and patient records be maintained confidentially.
- Reasonable response to any request you make for service including transfer, evaluation, or referral to another physician. We may not transfer you to another hospital until you have received information about your need for transfer and agreed to transfer.
- Knowledge regarding any healthcare affiliation or professional relationships as it effects your care.
- Be informed of any experimentation affecting your care. You may refuse to participate in any experimental care/research.
- Expect continuity of care and be informed in advance of what follow-up care or service is needed after discharge.
- Examine and receive an explanation of your bill-regardless of the source of payment.
- Be told what hospital rules and regulations apply to you as a patient.
- Formulate an advance directive and designate a healthcare surrogate to speak for you on your behalf. HMH will comply with your directives or provide appropriate transfer to another doctor or healthcare facility if HMH can not respect your requests.
